During the past decade, within a resistant political climate, the largest reforms in Medicare since its passage in 1965 were enacted into law. One of these independent and partial reforms affects hospital services, while the other affects physicians' fees. Together these reforms have transformed relations between the government and the providers, yet they have only a limited prospect for success. In Paying for Medicare, the author treats these reforms as "case studies" in policy development and analysis and demonstrates how substantive policy has been shaped by American political and administrative institutions. The studies are examined within the larger contexts of historic past, institutional successes and failures, and the contemporary "crisis" in health care.
